Return to sport and physical activity in young adults with ongoing symptoms 9-36 months after ACL reconstruction

Introduction: The relatively low return to sport rate (~50%) after anterior cruciate ligament reconstruction (ACLR) is well-established – not everyone recovers completely. Over one-third of people report ongoing symptoms at 2-6 years post-ACLR. Sport and physical activity participation in young adults with ACLR known, but may impact general health outcomes (e.g., weight gain, co-morbidities). Methods: Baseline data from the first 100 participants (age 30±6 years, 36% female, BMI 27.5±5.3kg.m-2) enrolled SUPER-Knee clinical trial (exercise-therapy education vs. minimal intervention control post-ACLR) were analysed. Main inclusion criteria aged 18-40 post-ACLR, discharged physiotherapy a symptomatic knee (Knee injury Osteoarthritis Outcome Score [KOOS4] <80/100). Pre-injury collected Tegner scale self-report questionnaire, which also assessed completed. Post-ACLR QoL was ACL-QoL questionnaire (0=worst, 100=best). Changes pre-injury non-parametric tests; logistic linear regression models examined between sport/physical number sessions attended QoL, respectively. Results: In year prior ACL injury, most (85%) played competitive (Tegner median 7, range 1-10); football (n=37), basketball/netball (n=29), other (n=19). (30±7 post-ACLR), decreased significantly 5, 0-9; p<0.0001), much fewer (20%) playing sport; (n=5), (n=6), (n=9). Only 13% returned same level sport. Pre-injury, 77% regular (≥30mins duration, ≥2x/wk walk n=30, run n=23, swim n=9, cycle n=14, gym n=43). Post-ACLR, more walked regularly (n=40), participated activities (run n=4, n=11, n=34).
